Medical Expense Reimbursement — General Procedure and Eligibility

Requests for reimbursement of medical expenses are to be presented through the Heads of the respective MDAs and addressed to the Honourable Commissioner or Permanent Secretary, Ministry of Health. Applications should include a photocopy of the applicant's staff identity card, relevant documents and evidence of payment. Reimbursement is limited to the applicant, spouse and not more than four children below 18 years of age as at the last birth date. Applications are addressed to the Lagos State Ministry of Health for consideration and recommendation; amounts within the approval limits of the Honourable Commissioner and Permanent Secretary, Ministry of Health are processed at that level, while others are forwarded to the Honourable Commissioner, Ministry of Economic Planning and Budget or the Governor, as applicable.
