Reimbursement for Drugs and Laboratory Tests

Applications for reimbursement of drugs and laboratory investigations unavailable at Government Hospitals must include a photocopy of the doctor's prescription clearly bearing the name and signature of a qualified Doctor from a State health facility. Prescriptions from private hospitals must be countersigned by a Doctor in the Lagos State Civil Service. Copies of receipts and other documents evidencing purchase or payment are also required.
